Agency on Aging-Area 4 funds and monitors older adult programs in seven counties in Northern California.  The following contracted services are provided in Yolo County:  Caregiver Support, Health Insurance Counseling, Information & Assistance, Job Readiness, Legal Services, Long Term Care Ombudsman Services/Elder Abuse Prevention, Nutrition, and Transportation. AAA4 also provides advocacy for senior programs and services. Referral Requirements: Age 60+ (Caregiver/care receiver exception) & Resident of Yolo County

LONG-TERM CARE OMBUDSMAN SERVICES

Ombudsman Services provides advocacy services to residents in long-term care facilities (Skilled Nursing Facilities and Residential Care Facilities for the Elderly). Ombudsmen identify, receive, investigate, and resolve problems on behalf of residents. The ombudsman program also provides community education and training, consultations and information, maintains a presence in long-term care facilities, and witnesses advanced health care directives for residents who reside in Skilled Nursing Facilities.
